The time is Autumn and the school year has just begun. The little one-room schoolhouse, where all grades were taught by one dedicated teacher, was a very important part of Victorian America. This little boy, enthralled with his new teacher, presents her with an apple, hoping to show his fondness. The little girl, with similar ideas, finding her much prized gift already being given stands a little insecurely in the background waiting her turn to present her token of affection. What neither of the children knows is that the teacher is holding still another apple behind her back and yet another is on the floor behind her.

As was the custom of the time, the doctor's office was usually in his home, and the more successful and affluent the doctor, the more elegant the home. Here, the mother of the little lad is determined that her young son is hoping to visit the doctor. He is equally reluctant to go. With his friend, the bear, in hand for security, it is evident that he has lost the battle.

Little boys often do not look forward to visiting the doctor as you can see with the little boy with his teddy bear in hand for support.  The mother is determined he will go, and the person who turns all this around is the welcoming nurse.

She is indispensable to the physician with her competence and expertise and an angel of mercy to her patients as she dispenses equal measures of compassion and caring.  Little children sense this and she is the medicine that makes the pill go down.
